Washington — The Chemical Safety Board has released additional information on 13 serious chemical releases as part of what the agency has called an ongoing agency effort to boost transparency.
CSB is sharing incident summaries, probable causes and other details that go beyond the requirements set out in the agency’s accidental release reporting rule.
The incidents covered in the agency’s fourth volume of expanded incident summaries occurred across seven states between July 2020 and this past May. They resulted in two deaths, 10 serious injuries and more than $1 billion in property damage.
CSB unveiled an initial compilation of expanded information, covering 26 serious chemical releases, in January 2025. The agency released a second volume, related to 25 incidents, in March and a third volume, offering added information on 30 incidents, in July.
In all, the incidents featured within the four reports led to 16 deaths, 75 serious injuries and more than $4.5 billion in property damage.
